FORM offers you a deeper dive into discipleship in the context of a welcoming community, where you can come to know Christ more intimately and learn to apply the riches of the gospel in everyday life, becoming a disciple who is ready to make disciples.

You are in the right place! The FORM leadership team welcomes you to journey with us as we share and practice key concepts of missional discipleship designed to equip you for a lifetime of following Jesus his way in your unique context.

The 2024 winter session of FORM begins January 21 and ends April 21, with commencement and commissioning recognition on Sunday, April 28, with breaks as listed in the FAQ document linked below.  

FORM Meets: Sundays from 6:00 - 8:30 p.m. and Thursdays from 6:00 - 8:30 p.m. Regularly weekly participation includes two meetings that incorporate teaching and training, coaching huddles, and prayer together. We have a few special events that occur on Saturdays, which you may note in the FAQ document linked below.

FORM FEATURES

Spiritual Formation

You will be trained to live into what is wholly true of you because of Christ.
FORM helps you discover your true identity in Christ and what that means for your life as you learn to confidently embrace your life and calling. You also will discover how to participate in spiritual rhythms in various expressions of community.

Leadership Development

Your immersion in this discipling culture will equip you to engage in service and training opportunities you encounter and train you to become a disciple who wants and is able to make disciples.

Biblical Theology

FORM will help you develop a firm grasp of God’s character and the biblical story of his work in the world as you discover explore key themes throughout scripture.

Missional Training

FORM will help you gain confidence to live an intentionally missional lifestyle no matter where you live, work, or play.

FORM Expectations


Be Present. Our time together is most beneficial when everyone fully engages and participates in the process. You are expected to participate at Apex on Sunday and Thursday evenings during scheduled meeting times in addition to your flexible ministry hours. We ask you to let leaders know if you cannot attend for unforeseen circumstances (sickness, etc.) or you're out of town.

Be Punctual. Come prepared to start on time.

Be Teachable. Ask questions and stay curious! People who adopt the posture of a lifelong learner will benefit and learn more than the person who thinks they already have it all figured out.

Be Informed. Carefully read all emails and texts from FORM leaders so that you are well-informed. Respond in a timely manner when necessary.

Be Prepared. Complete assignments ahead of time and come prepared to participate in our meeting discussions.

Be Positive. Your attitude is contagious! Negativity not only hurts you, but impacts the rest of the team. Take time to relax and refresh your spirit. If you encounter a problem, make sure you are handling conflict biblically. Your FORM leaders are there to help you.

Be Honest. We’ll dive deep in our time together and we want to get to know the real you. To fully invest in your development requires honesty and openness about where you’ve been, where you are, and where you’re headed. Every effort is made by the FORM leaders to create and nurture a safe space for everyone to practice transparency.

Be Flexible. Things may not go as planned! We encourage innovation and creativity in problem-solving. Ask questions and be prepared to adapt to situations as they unfold.
